When seniors and their families experience care challenges coupled with other related life stressors, it’s not uncommon for them to seek out answers and support. Tracy’s great passion is helping seniors and families receive the help and guidance they need in these times of crisis.

Tracy has been an RN for over 30 years, having begun her nursing career in the Surgical ICU. Her nursing and leadership skills were quickly realized and she was elevated to a supervisory role at the young age of 24. Tracy went on to combine her nurse management skills with her entrepreneurial spirit. She was the Founder of a 10,000 square foot Award Winning Med Spa in the Seacoast area of New Hampshire.

    

For the past 17 years Tracy has focused her passion and nursing expertise in the care of seniors and families. She has served as the Director of Nursing and the Executive Director of Independent, Assisted and Memory Care Communities in the Buckhead area of Atlanta.

​

Tracy is caring in her approach and able to bring solutions to complex care challenges that greatly improve the well-being of seniors and their families. She has a wealth of experience as it relates to the care of seniors and specialized training for seniors experiencing the overwhelming effects of dementia.

​

Tracy works closely with her talented team to offer a wide range of resources and solutions to seniors, families and caregivers that maximize the quality and enjoyment of life.

​

Tracy is a wife of 32 years and a mother to 3 amazing daughters and a wonderful son-in-law, as well as 6 beautiful grandchildren. Tracy enjoys the ocean, playing the piano, painting and cooking gourmet family meals.

Janet has a lifelong passion of care and advocacy for seniors since becoming a nurse in 1982. She began her career with bedside nursing and advanced to become a specialist in cardiology and diabetic teaching.  Janet then became a cardiac nurse specialist in cardiopulmonary rehabilitation and coordinated the cardiac education program for 11 years. 

​

In 2003, she became a geriatric care manager and worked for both large and small care management companies in south Florida.  Moving to Atlanta in 2007, she continued her work in care management helping her clients and their families coordinate and navigate through the healthcare maze. 

​

Janet utilizes her success and experience in senior care management and she brings valuable knowledge and solutions that greatly improve the health and quality of life of her clients. The common thread throughout Janet’s career has been her passion for advocacy and education. Janet is a Certified Care Manager  with the National Academy of Certified Care Managers and an Advanced Professional Member of the Aging Life Care Association.

​

Janet and her husband Brad enjoy camping in their RV, traveling and spending time with their two adult daughters. 

Melissa Hyatt, RN, CMC

Melissa is a compassionate Aging Life Care Manager with more than 30 years’ experience in the health care arena.  Throughout her career, she has worked in hospitals, home health agencies, hospice care, and case management.  One of Melissa’s favorite and successful endeavors was creating and directing an Atlanta community based program to support older adults living at home. Following that, she served for over eight years as the Executive Director for an Assisted Living and Dementia Care Community in Johns Creek.

 

Melissa embraces her role as a geriatric care manager with great enthusiasm and tries to bring a bit of humor and joy to her clients each time they are together. Her mutual respect and love for seniors began at a young age while helping care for her grandparents and other family members. This was the beginning of her lifelong passion for working with and learning from older adults. Throughout her high school years, she regularly volunteered at local senior centers, most often singing and playing her guitar. 

 

Tapping into her expertise as a Registered Nurse with specialized knowledge of chronic disease management and dementia care, Melissa partners with her clients and their families to help guide and educate them through the often complex challenges of aging. She helps them make informed decisions in order to achieve quality care, peace of mind, and optimal outcomes. Using a client-centered and collaborative approach, Melissa is committed to assisting clients find a successful blend of individual choice, safety, and available resources.

 

Melissa was born and raised in upstate New York. She attended nursing school in the Boston area and then lived there for 18 years. She moved to Georgia in 1997 where she raised her two sons. She loves being a new grandmother and also enjoys music, movies, cooking, playing guitar, and spending quality time with her family and friends.

Lauren Voyles, RN, BSN, CCM

Certified Patient Advocate

Lauren has an extensive and varied background navigating healthcare for clients with a variety of chronic and acute diseases. Her particular focus includes case management with medical oversight for clients with injury, cancer, motor system disorders, autism and heart failure.

​

Lauren is a gifted advocate, educator and Certified Care Manager who understands the social and emotional component that elder care places on the family. With her experience and insight, Lauren provides clarity to help families proactively navigate the often-stressful medical events that can impact family and home life.

​

Additionally, herself a cancer survivor, Lauren intimately understands the vulnerabilities and complexities of illness and the maze of medical care. These experiences have guided her to advocate for her clients with passion, purpose and success.

​

Lauren holds a RN, BSN through the University of Oklahoma and is an Alfus Certified Patient Advocate through the University of Miami.  She is a Certified Care Manager with The National Academy of Certified Care Managers. She also has advanced training in Gerontology through the University of Georgia.

​

In her free time, Lauren enjoys volunteering at her church and time in the Georgia Blue Ridge Mountains with her husband of 23 years and two teenage daughters.

Mandy Antonio, RN, BSN

Mandy Antonio is a healthcare veteran who brings to Premier Care Management and her clients a record of exceptional leadership, proven outcomes, and a passion for excellence. With 20 years of experience as a Registered Nurse, Mandy’s nursing career has allowed her to pursue many different avenues of care, including hospital-based nursing, travel nursing, high-tech in-home pediatric nursing, home health, hospice, and senior living care. Through all of these experiences, her unwavering desire for patient advocacy and compassionate care have remained core attributes.

 

With Mandy’s broad range of healthcare contributions this has allowed her to empower her families through communication, education, out-of-the-box thinking, and personalized attention while creating solutions and allowing execution of informed decisions. As a result, assisting families and their loved ones in navigating the complexities of care they encounter is Mandy’s true love.

 

Mandy was born in Cleveland, Ohio, yet spent her childhood growing up in Stone Mountain and Alpharetta, Georgia. She continued to extend her Georgia roots by attending Georgia State University where she graduated Magna Cum Laude and earned her Bachelor of Science in Nursing. She currently resides in Roswell, Georgia with her husband of 10 years, Patrick, and their 2 sweet spaniels, Keona and Lily.

 

Mandy enjoys the beach, traveling, cultivating both her flower and vegetable gardens, reading, and enjoying various types of cuisines.

Dana Chapman, BSW, CMC

Dana credits a beloved social work professor and her civic minded parents for guiding her to her first internship in a small rural nursing home.  She began working in special education and juvenile probation before spending the next fifteen years in Victim Advocacy.  While serving a community rape crisis center she was honored with the Georgia State Governor’s Criminal Justice Coordinating Council Eagle Award for Outstanding Advocacy.

​

Her early career included hospital social work, elder care management, hospice social work, guardian ad litem work for Superior Courts and co-leading recovery groups for addiction and domestic violence victims.

​

In 2011 Dana became the Executive Director for The Guest House, Inc a nonprofit Day Care and Activities program for senior adults and those with dementia. After six years of service there she became the very first Executive Director for a Hall County nonprofit specialty transportation service for seniors who no longer drive.

​

Earning her Certificate in Care Management in October of 2020, Dana is thrilled to be working with Premier Care Management of Georgia in service to seniors and their families.  Dana uses her extensive community connections, rich work experience and tender heart to help seniors and their families navigate, plan and maximize available services and support for older adults.

​

Dana is deeply involved in her community, through nonprofit boards and volunteering.  She was honored by Woman Source, a career and personal development network, with their 2014 Community Hero Award.  Currently she serves on the Executive Board for the North Georgia Senior Resource Network and is an active Gainesville Kiwanis Club member.

​

Dana is blessed with two children, two grandchildren and extended family around the country. She and her husband enjoy spending time with their two rescue papillon mix pups.

Angela Jones Daniel, RN, MSN, MBA

Identifying challenges and helping people solve them has been a passion of Angela's for years. She believes that the real challenges in life is helping people recognize that they are not in their situation alone, but rather have resources and help available that they didnt even know existed. This is never truer than with our senior population.

​

Angela has been a registered nurse for over 30 years. She has served the home care industry as both a Director of Clinical Operations and Vice President of Clinical Operatlions where she was able to decrease hospitalizations and ER visits by 50% in one of our most vunerable populations.

​

Angela is empathetic and caring in her approach. She recognizes that a collaborative partnership is the best path to optimal care. She is bless and honored to be able to assist families on a path that encourages health and wellbeing.

​

Angela is an avid traveler, enjoys her adorable cats, Sushi and Bubsey and when not at work she's on the dance floor learning how to perfect the salsa dance steps.

Monique York, RN, CDP
Geriatric Care Manager


 

Monique has been in the healthcare field for eighteen years and is a graduate of North Georgia College and State University, where she received her Bachelor of Science in Nursing in 2003.   She has expertise in several areas of nursing to include Neonatal and Pediatric ICU but has spent the last 6 years of her nursing career in Hospice. 

​

While working with seniors in Hospice, she realized how important good senior care was to this population and soon realized that senior care was where her heart was.  She believes that every senior should be cared for with compassion and respect and therefore she strives to ensure that this happens.  She believes that no senior should ever feel alone to navigate through medical and community resources and therefore believes that Senior care management is such an important service.

​

Monique was born in Millington, Tennessee but spent most of her childhood growing up in Ulysses, Pennsylvania.  Right out of high school, in 1989, she joined the United States Air Force.  She was stationed at Ellsworth AFB, where she worked on Offensive Avionic systems on the B-I Bomber.  She stayed in the service for eleven years before becoming a registered nurse.

​

Monique enjoys traveling to anywhere near the ocean, singing, and spending time with her husband Tracy and their four children.

Brenda has always known that she would be a social worker. Being a good listener and helping others has been her life-long driving force. She grew up as a military “brat” as her father served in the United States Air Force for 23 years. The family moved many times due to her father being stationed throughout the East Coast, Midwest, and Europe. Her upbringing taught Brenda to adapt quickly, get to know other people and their cultures, and discover that family is a constant when everything else around her was changing.

​

She graduated in 1994 with a Bachelor of Social Work degree from Northern Michigan University. Brenda found she had an affinity for working with the elderly population and wanted to make that her life focus. She believed then, as she does now, in looking at “problems” as challenges that can be overcome with perseverance and by thinking outside the box.

​

After graduating, she worked as a Care Manager at the Area Agency on Aging in the Upper Peninsula of Michigan before she and her husband relocated to Georgia in 1996. For a time, she worked as a hospital social worker and, for the past 25 years, has served as the director of social services in two nursing homes. Her experience in long-term care and as a facilitator of an Alzheimer’s support group for 20 years has provided Brenda with many opportunities to help seniors access skilled nursing services, return home with appropriate community services, and assist with end-of-life transitions at home and at facilities. She believes helping seniors live their best life is truly a gift.

​

Brenda and her husband live in Douglas County. They have two adult daughters and two dogs. She enjoys listening to all types of music, watching football and soccer, baking, and reading

Leah Is a 1990 graduate of Brenau University with a Bachelor of Science in Nursing.  She found her love of geriatric patients early in her career and has given the last 32 years to caring for seniors. 

 

Leah has been a Registered Nurse at the bedside and in leadership roles.  She is a Licensed Nursing Home Administrator and works with patients and families to manage care in various settings.  Leah is an advocate for her patients by networking area resources to support their needs and wishes.  

​

Leah lives in northeast Georgia with her husband, Jody.  Their family includes three beautiful daughters, one son in law and the most “GRAND” grandson, Vann.   She has been devoted to service in her church serving as organist for greater than 20 years

Barbara always knew she wanted to become a Registered Nurse. As a senior in high school, Barbara went to evening classes to become a Certified Nursing Assistant (CNA). This allowed her the opportunity to work as a CNA in a local Skilled Nursing Facility while she attended nursing school. Barbara loved working with geriatric patients and spent several years caring for them. As a Registered Nurse, Barbara had a wonderful opportunity to work in Case Management with complex pediatric patients. Barbara was quickly appointed Director of Case Management and Patient Access, ensuring the department of Nurse Care Managers provided high quality and empathetic care. In 2017, Barbara relocated to Atlanta where she accepted the position of Director, Care Coordination for a prestige academic medical center. Barbara was entrusted to lead a team of Social Workers and Nurse Case Managers who serve our most vulnerable geriatric patients, ensuring effective transitions of care, care coordination, and high-quality outcomes. 

​

Barbara's true passion is navigating complex situations while keeping the client and family at the heart of it all. She loves having the ability to connect with clients and families, and build strong trusting relationships. Barbara's soft and gentle approach paired with her innate ability to problem solve and take action is what sets her apart.  Her many years of experience working in Care Management coupled with her knowledge and passion for seniors and the aging process has made her a great match for the countless lives she has been able to touch along her journey. 

​

Barbara was born and raised in East Hampton, CT.  While she is a CT native, she is enjoying her life in the south with her husband, Mark and their two daughters, Mackenzie and Madison. Barbara loves the beach, to travel, and to spend time with her family!

Jennifer has been a Registered Nurse for 10 years and is a graduate of Brenau University's School of Nursing where she received her Bachelor of Science in Nursing.

​

Jennifer began her career as a Medical-Surgical Nurse where she developed her nursing skills and hands on experience. This experience as a Med-Surg Nurse laid a strong foundation for her to work as an Intensive Care Nurse. During her time working in the ICU, she became Trauma Certified and worked with critically ill trauma patients. This time in the ICU developed an unwavering passion and desire to advocate for her patients throughout their hospital stay and beyond. Jennifer worked closely with the hospital social work team communicating what her patient's needs would be upon discharge home. 

​

With her experience as a registered nurse, Jennifer views her work as a focused mission to serve others, especially older adults. Her personal and professional confidence is contagious as she partners with families and clients to tackle complex medical and care issues. She is passionate about empowering older adults as they navigate the continuum of care that exists as part of the aging journey.

​

Jennifer calls Georgia home as this is where she has spent most of her life and where she and her husband of 25 years raised their three beautiful girls. She enjoys time with family and their Bullmastiff dog named Hank that is attached to her hip.  Jennifer enjoys hiking the great state of Georgia, Olympic weightlifting, and spoiling her granddaughter rotten.

Cassandra was born in Nashville, Tennessee, and has lived in Georgia for over 13 years.  She is blessed to share life with her husband, children, grandchildren, and those that she can assist.

​

Cassandra has always had a passion for helping others. The opportunity of assisting her vision-impaired grandmother for many years. Allowed her to develop a concern and passion to help other families with life-changing decision-making for over 25 years.
 

Her journey started early in case management assisting as a court liaison for Kelly Miller Smith in 1993.  She was able to see a greater need and desire to assist families with different situations from all walks of life.  
 

After she realized there was a decline in follow-up, well-checks, companionship, and essential care. Cassandra found that she wanted to make herself available to bring hope for change to these areas.
 

On her journey to do so, she has been able to assist families as a youth counselor, mentor, life coach, senior care, and special needs and is pastoral licensed and ordained.
 

Cassandra found that she was now on the right path assisting families that deal with planned or unplanned life situations.  Being a part of Premier Care Management has allowed her passion, love, and patience to assist seniors even more.
 

To experience seniors laughing and sharing life stories and even allowing opportunities for additional help brings a great joy to her heart.  
 

Cassandra connects with her community by using her talents and gifts to serve others. Cassandra has been recognized and awarded throughout the years as she has been blessed to work in her passion.
 

Cassandra enjoys traveling, reading, nature walks, and decorating.

Susan Phillips

Susan has been with Premier Care Management since its inception in February of 2018.  She currently serves as our Senior Move Manager and also enjoys her work as a Dementia Activity Specialist, and care manager assistant.

​

She has a special gift of relating to seniors in a variety of different circumstances and settings. Susan works with dementia clients both at home and in the senior living setting. She is skilled at engaging  seniors in activities they have enjoyed in past. These can range from shopping, concerts, eating out etc. She achieves the goal of adding positive stimulation and enjoyment during the senior’s day. Susan also assists our RN Care Managers when needed.

​

As a Senior Move Manager, Susan is skilled and experienced at managing a senior’s move on all levels.  Each client’s needs are carefully assessed and noted, and special care is taken to make the new home transition as smooth as possible.  She seamlessly coordinates all aspects of the move from beginning to end.  Susan makes a profound difference in the often-difficult moving process, allowing our senior clients to experience a move that is as stress free and pleasant as possible. 

​

Susan and her husband David have three sons, twins and an older brother all in college.  They direct the National Clogging Association in their spare time.  Susan enjoys junking as a favorite pastime where she travels.

Cheryl Hess
Director of Strategic Partnerships

Cheryl began her legal career in 1985, entering corporate law and working her way up to managing a large corporate legal department. She then transitioned to governmental affairs, where she worked with state and federal legislative staff. After starting a family, Cheryl decided to embark on a career in real estate and construction, which allowed her to interact on a more personal level with clients, in addition to her management responsibilities.

​

Experiencing firsthand her grandmother’s battle with dementia, Cheryl decided to put her legal skills to work not only to help her grandmother, but to help others in similar situations. As an elder law paralegal, she has assisted countless families navigate through some of the most difficult times of their lives. Cheryl’s passion and enthusiasm for helping others with the challenges of aging is a driving force in her life. Cheryl now joins Premier Care Management with a focus on enhancing client and professional partner relationships.

​

Cheryl resides in Chino, California with her husband and their three children. Her love of traveling takes her around the world, but she is happiest at home, cooking and laughing with her family and friends.

Peggy is a native of Atlanta, having moved north of the city to beautiful Dahlonega in 2000. She spans a 35 year career with extensive experience with bookeeping, government benefits and business management. Peggy also worked as a licensed real estate agent and managed the family sign business. In 2009, she began working in the nursing home industry discovering her love for seniors. Peggy developed an expertise in Medicaid assisting with both applications and renewals.

 

She helps our seniors currenly with their daily money management needs as well as in the processing of medicaid applications.

​

Peggy has had several family members that she helped through the aging journey. She experienced first hand the stress of caregiving. She brings an understanding and empathy to the relationship that leaves the senior feeling confident that they have found the help they need. 

​

Peggy attends car shows around the country with Bobby her soul-mate of over 20 years. She loves to travel, especially to visit her second home in Alaska. 

Keith Johnson
Director of Marketing
& Creative Development

Keith is a marketing leader with over 25 years of experience as an entrepreneur and business owner with a focus in the personal care/service industries.  Keith joined Premier Care Management at its inception in February of 2018 to focusing on branding, market strategy and overall creative development.

​

Before joining Premier Care Management Keith lead multiple marketing and business development initiatives within his own companies.  A native New Englander, Keith first made his mark in business as a top salon and spa owner with locations on Boston’s prestigious Newbury Street in the Back-Bay area of Boston. Keith later joined forces with his wife Tracy to create and The Wingate Salon & Spa in the Seacoast area of New Hampshire. The Wingate became one of New England’s premier Award-Winning Spas voted Top 200 Fastest Growing in North America six consecutive times by Salon Today Magazine. Keith is also a Nationally recognized professional sculptor. His life-size portrait sculptures have appeared in such notable places as The Ronald Reagan Presidential Library, The Basketball Hall of Fame, and The John F. Kennedy Medical Center.

​

With his years of marketing experience and his uncompromising commitment to excellence, Keith continues to explore new opportunities to innovate and grow the company.  But he is always quick to say,  “The true success of Premier Care Management is found in the passion and talent of our growing team of RN Care Managers and Administrative Professionals. In his free time Keith enjoys RV camping on the ocean, fine art, gourmet cooking and spending time with his wife of 32 years, his three children and six grandchildren.

Michael O'Brien

Michael has worked in finance with senior living communities as a Business Office Manager and Director of Finance for the past 7 years. He is a member of the American Association of Daily Money Managers. He loves numbers and enjoys creating and managing budgets. He also loves helping seniors and when not behind his desk, you can find him spending time talking with seniors and their families. He has led community events and is very detailed oriented with an eye for excellence.

Michael is an avid football fan, loves fishing, contributes to his church as a Board Member and loves making homemade pizza for friends and family.

As a father of 6 children under the age of 8, Michael and his wife Ashley spend much of their free time enjoying family activities and making memories.

Kevin leverages professional and finance skills honed over more than 7 years of experience. He has managed accounts for Fortune 500 companies but also enjoys daily money management services working with variety of individual clients.

Kevin enjoys solving problems and has a real passion for helping others. He is skilled at helping clients balance accounts and oversee finances and budgets. He strives to meet goals set forth when it comes to financing and money management.

​

Kevin is a Business Management graduate who is an avid football and baseball fan. He loves home projects and enjoys traveling to new destinations. 

 

As a husband and a father of a two-year-old, Kevin and his wife Kaylie enjoy family time and are always looking for fun ways to create lasting memories.
