Job Title:  Geriatric Care Manager​

PremierCMGA is seeking full-time RN, LCSW, or Geriatric Care Managers who can deliver high quality personalized assessments and care management services to enhance the health and well-being of our clients. 

​

Key Responsibilities:

​

  • Able to effectively represent PremierCMGA with prospective clients and families, in order to engage clients and families in care management services.

  • Work within existing family systems to ensure client's needs are met and families have the tools needed to communicate and work towards realistic goals.

  • Responds to all clients and prospective clients promptly, sets up home visit and assists clients and/or families through the intake process.

  • Implements a thorough assessment in the senior’s home, which includes completing a goal-oriented care plan that is given to the client and his/her family.

  • Provides comprehensive, personalized care management services, based on the assessment and the PremierCMGA protocols and standards for service.

  • Provides appropriate levels of care management service based on the intensity of client need and client and family preference.

  • Provides supportive coaching, educational and advocacy services as required.

  • Consistently establishes a strong relationships and excellent rapport with clients, families, referral sources and other community agencies and resources in the greater Atlanta-Atlanta North region in order to effectively facilitate the care management process.

  • Uses PremierCMGA’s online record keeping system to complete all charting on a daily basis.

  • Updates care plans on a regular basis in response to changing needs of the client.  

  • Complies with all client billing and registration procedures, including accurate timekeeping for program billing purposes.

Physical Demands:

​

  • Sitting, walking, and/or standing for brief periods of time.

  • May be required to lift up to 20 pounds.

  • Occasional reaching and/or grasping using hands and/or arms.

  • Regular driving of an automobile.

  • May be required to ascend/descend stairs.

  • Regular use of hands requiring dexterity in using the telephone, computer keyboard or mouse.

  • Visual ability to read documents and computer monitor.

  • Exposure to low to moderate noise levels characteristic of an office or home environment.

​

Job Qualifications:

​

  • Active State of Georgia Registered Professional Nurse or Social Work License preferred.

  • Preferred Care or Case Management Certification, or Gerontology Certificate.

  • Minimum Experience: At least 10 years of experience working with the disabled and elderly population.

  • A proven track record of employment and success in care management.

  • Working knowledge of community health/support resources and delivery systems.

  • Excellent interpersonal skills and ability to form caring and professional relationships with seniors and their families.

  • Self-disciplined with excellent analytical and problem solving skills.

  • Excellent verbal and written communication skills.

  • Basic computer proficiency as an important charting, tracking and planning tool.

  • Self-motivated with the ability to work independently and as part of a collaborative team.

  • Ability to work well with others, including staff, clients, members and the public.

  • Car and current driver’s license and current automobile insurance.
