Tracy Sachs

Geriatric Care Manager

Tracy is a compassionate Registered Nurse and care manager with more than 20 years of combined experience in nursing, social services, hospice, dementia care, and healthcare coordination. She is dedicated to helping older adults and their families navigate the medical, emotional, and practical challenges of aging.

Her passion for elder care began after her father's cancer diagnosis, an experience that profoundly shaped her understanding of the importance of compassionate guidance and advocacy during difficult seasons of life.

Tracy earned her Bachelor of Arts degree in Sociology from Miami University, with a minor in Gerontology and a concentration in Social Work. She began her career in social services, working closely with older adults and their families. She later served as Director of Social Services in skilled nursing rehabilitation and Alzheimer's care settings, gaining extensive experience in dementia care, discharge planning, family support, and healthcare coordination. She also served as Director of Admissions and Marketing for a skilled nursing facility, helping families understand care options and navigate complex healthcare decisions.

While raising her three children, Tracy returned to school to pursue nursing as a second career. As a Registered Nurse with more than 15 years of experience, she has worked in postpartum care, newborn nursery, surgical services, transplant services, hospice, and RN case management. Her clinical and hospice experience reinforced her belief that families need both exceptional medical care and a trusted advocate to guide them through challenging situations.

Tracy also understands caregiving from personal experience. While helping care for her elderly aunt during kidney failure, she coordinated appointments, communicated with providers, and helped her family navigate a complicated healthcare system.

Known for her calm demeanor, reassuring presence, and strong advocacy skills, Tracy is committed to helping families feel informed, supported, and empowered. Outside of work, she enjoys traveling, interior design, cooking, and spending time with family and friends. Her three adult children remain her greatest joy and inspiration.
